HarborWell Health SMS / Text Message Program — Consent & Disclosures
ABOUT THE PROGRAM
HarborWell Health, a physician-led care coordination provider, sends
transactional, care-related text messages to enrolled patients of
contracted health plans. Two types of messages are sent: appointment
reminders (with a text-back confirmation option) and alerts when you
have a new message in your secure patient portal. We never send
marketing or promotional texts.
HOW WE OBTAIN CONSENT
We only text patients who have given express verbal consent directly to
HarborWell Health during a recorded phone call with a care coordinator,
before any message is sent. Every patient is read the following script,
word for word:
"Would you like to receive automated text messages from HarborWell
Health with appointment reminders and secure patient portal alerts?
Up to 2 messages per appointment; frequency varies with your care.
Message and data rates may apply. Reply STOP to opt out, HELP for
help. Consent is not a condition of care. Do you agree?"
Only a clear "yes," captured on a recorded line, enrolls a patient. The
date, time, and staff member are documented in the patient record and
retained for at least four years. Patients who consent receive a
one-time text confirming their enrollment.
PROGRAM DISCLOSURES
- Message frequency varies with your care; typically up to 2 messages
per scheduled appointment, plus occasional patient portal alerts.
- Message and data rates may apply.
- Reply STOP to any message to opt out at any time. You will receive
one final message confirming you have been unsubscribed.
- Reply HELP to any message, or email support@harborwellhealth.org,
for assistance.
- Consent is optional and is not a condition of receiving care.
PRIVACY
No mobile information will be shared with third parties or affiliates
